I am Vladimir Hachinski,
Distinguished University Professor at Western University in London, Canada.
The topic today is "The Expanding Frontiers of Cerebrovascular Disease".

Expansion in numbers, expansion in diagnostic categories,
and expansion into the microcirculation.

In terms of expansion in numbers,
the absolute numbers of strokes occurring in the world are rising.
Moreover, the lifetime risk of stroke from age 25 years onwards is 24.9 percent.
Unfortunately, this is an increase from 1990-2016 from 22 percent to 24 percent,
and these risks differ across the world.
The highest is in East Asia,
38.8 percent, next is Central Europe,
31.7 percent, and then Eastern Europe
31.6 percent and the lowest risk is in Eastern Sub-Saharan Africa.

By now, neurological disorders are the leading causes of disability adjusted years.
DALY's, disability adjusted years are
a combined measure of premature death and years spent with disability.
More than half of the neurological disorders are accounted for by stroke 42 percent,
and dementia 10 percent,
giving a percentage of 52 percent of causes of
disability adjusted life years and
hence the single most important topic in terms of what can be done about these disorders,
given that they share the same risk factors.
