This overview addresses clinical trial endpoints with particular emphasis on the fundamental role of clinical trial endpoints in evaluating new therapies and their influence on regulatory approval and clinical practice. We will discuss the types of endpoints, primary, secondary, and exploratory, their definitions, and why accurate pre specification is critical. The lecture will examine the use and validation of surrogate endpoints and explain the importance of distinguishing between statistical and clinical significance. Lastly, we will highlight how thoughtful endpoint selection impacts trial design, regulatory review, and advances patient care. Clinical trial endpoints are a foundational aspect of clinical research, central to evaluating new therapies. Endpoints are how we measure success in trials and influence regulatory approval, clinical adoption, and patient care. They can vary widely from tumor response rates in oncology to asthma exacerbations or quality of life measures. In this lecture, we'll explore types of endpoints, their definitions, regulatory uses, and key limitations investigators must consider in study design. Every well designed clinical trial defines clear primary, secondary, and exploratory endpoints. The primary endpoint is the main outcome a trial is powered to measure, like overall survival in cancer or HBO one C reduction in diabetes. It guides the sample size and statistical plan.
