Disseminating the TogetherTeam Virtual Connected Care (VCC) innovation system-wide: From ideation to implementation

Abstract

Workforce shortages, exacerbated by the COVID-19 pandemic, necessitated the development of novel care delivery models that could leverage the available nursing workforce in concert with emerging technology. One large national healthcare system successfully presented a case for the implementation of an innovative model that would team available nurses, a dedicated care partner and leverage technology to deliver adult acute care in inpatient units. The model was developed, prototyped and tested, with outcomes catalysing the system decision to scale and sustain the model across all acute care hospitals. A different type of implementation team with a focus on change methodology was organised, established upon the important values of the organisation, and utilised to implement the model on an initial 2,500 beds. Principles of Human Caring Science, relational coordination, professional governance and the sustainment of health information technology structures were both cornerstones of the model and enablers of the change process. This paper, in a case study format, details the model, change tactics and learnings from the implementation. Author's Biography

Murielle Beene , DNP, MBA, MPH, MS, RN, NI-BC, PMP, FAMIA, FAAN is Senior Vice President and Chief Health Informatics Officer at Trinity Health, a national health system headquartered in Livonia, Michigan, across 27 states with 101 acute care hospitals, 126 continuing care locations, 136 urgent care locations and numerous ambulatory clinical practice sites. She is responsible for strategic and visionary leadership communicating complex trends to influence change improving the impact of informatics on the systems of care. Murielle advances activities of health informatics and analytics in support efficiency and effectiveness of healthcare operations with the primary goal of advancing evidence-based health practice through data, information and dissemination of knowledge and wisdom. Beene has over 27 years of health informatics and analytics experience in various professional positions. Prior to working for Trinity Health, she spent 10 years serving as Chief Nursing Informatics Officer at the U.S. Department of Veterans Affairs. Beene is a registered nurse who holds a doctor of nursing practice degree and a master of science in public health from the University of Massachusetts at Amherst. She also holds a master of nursing informatics degree from the University of Maryland and a master of business administration. Beene maintains certifications in Project Management from the Project Management Institute (PMP), Nursing Informatics from American Nursing Credentialing Center (NI-BC), Fellow of the American Medical Informatics Association (FAMIA) and the American Academy of Nursing (FAAN).

Gay Landstrom Gay L. Landstrom, PhD, RN, NEA-BC, FAONL, FACHE, FAAN, PCC, is Senior Vice President and Chief Nursing Officer for Trinity Health, a national health system headquartered in Livonia, Michigan, across 27 states with 101 acute care hospitals, 126 continuing care locations, 136 urgent care locations and numerous ambulatory clinical practice sites. Prior to returning to Trinity Health, Landstrom served as the Market Chief Nursing Officer for Ascension Michigan, the Executive Vice President and Chief Nursing Officer for Dartmouth-Hitchcock Health system in New Hampshire and Vermont, and the System Chief Nursing Officer for Trinity Health prior to 2014. Under her leadership, nursing professional practice has been hallmarked by outstanding quality and safety, leadership development, evidence-based practice, innovative care delivery models and the expansion of staff empowerment structures. Landstrom has served as an Appraiser for the American Nurses Credentialling Center’s Magnet Program Office of the American Nurses Credentialing Center since 2006. She was named an inaugural Fellow of the American Organization for Nursing Leadership. In 2023, she was inducted as a fellow of the American Academy of Nursing. Landstrom received her bachelor’s degree in nursing from Rush University in Chicago, her master’s degree in nursing administration from the University of Illinois at Chicago and her PhD in nursing from the University of Michigan. Her research interests include missed nursing care, safety climate, staffing effectiveness and implementation science.
