Abstract
Organisations, specifically in the healthcare industry, often find themselves in a state of ‘Reliability Divide’. This is a concept that the authors have developed to explain why most organisations never realise the culture change needed to become highly reliable. In an organisation there is a variable time frame to get to daily work. Because of the time involved, some of the front-line staff who participated in theory, academics and now operations have become mid- or lower senior level leaders. Along the journey they become the vector that infiltrates the organisation to reach Fabric. Multidirectional high reliability organisation (HRO) adoption starts to occur here, which further drives an organisation towards zero harm (nirvana).

Author's Biography
William Patterson Dr William Patterson serves as Chief Executive Officer of the VA Heartland Network/VISN 15, Kansas City. His interest in high reliability organisation (HRO) started in 2014 with a campaign entitled ‘Moving Beyond Performance Measures’. He led his network in Just Culture and Continuous Process Improvement training and development at that time; he now serves as VA National Co-Chair for the HRO effort. Prior leadership roles include Chief Medical Officer VISN 15 and Chief of Staff, Harry Truman VA Medical Center. Dr Patterson served on the faculties of the University of Missouri and the University of Kansas Medical Schools. He is a retired Army National Guard Colonel, a graduate of the Army War College and a veteran of both Iraq and Afghanistan.
David Isaacks serves as the Chief Executive Officer (CEO) of the North Florida/South Georgia Veterans Health System. The Health System is a 706-bed affiliated medical, speciality, surgical, psychiatric and research teaching system spanning two states and 50 counties. David previously served as CEO of Kansas City VA Medical Center, Kansas City, Missouri, and as CEO of Harry S. Truman Memorial Veterans’ Hospital in Columbia, Missouri. David is a Fellow of the American College of Healthcare Executives, an Excellence in Government Senior Fellow, a graduate of the Walter and Leonore Annenberg Leadership Institute — Class of 2008, the 19th Annual Prince Williams Regional Chamber of Commerce Valor Award winner from 2005, a 2003 U.S. Marine Corps Institute Graduate, certified LEAN Black Belt and a veteran of the United States Marine Corps.
