Northwell Health’s regional model: A blueprint for integrated care and operational success
Abstract
This paper analyses Northwell Health’s Eastern Market ambulatory care transformation as a strategic response to healthcare delivery challenges. By examining its regionalisation initiatives, healthcare executives can identify implementable frameworks for their own organisations. The research methodology includes a detailed assessment of Northwell’s structural reorganisation, collaborative care models, technological infrastructure investments and workforce engagement approaches. Findings reveal that strategic regionalisation produced measurable improvements in patient access, care coordination, provider satisfaction and operational efficiency. Success factors crucial to these outcomes include the establishment of defined governance structures, investment in patient navigation services, implementation of streamlined referral systems, development of community-based partnerships and prioritisation of workforce engagement strategies. This case study provides healthcare leaders with practical strategies to restructure their organisations while advancing the quadruple aim of health care. Joseph S. Baglio, MBA, is Senior Vice President of Eastern Market Ambulatory Services at Northwell Health. Prior to this role, he served as Vice President of Regional Ambulatory Finance, providing strategic and financial operations oversight for Eastern Market ambulatory operations, which includes 160+ practice locations spanning 1,500 square miles across Eastern Long Island.
Armando Castro-Tié, MD, is Senior Vice President, Eastern Market Physician Executive and Chair, Department of Surgery and Surgical Chief of South Shore University Hospital at Northwell Health. Prior to this role, Dr Castro served as the Chair, Department of Surgery at LIJ Forest Hills and Vice Chair for Northwell’s Surgery Service Line.
Cindy Maher is Vice President, Human Resources for Eastern Market Ambulatory at Northwell Health. Prior to joining Northwell, Cindy had more than 25 years of experience in Human Resources across various industries.
Jeffrey Musmacher, MBA, R.T.(T), is Assistant Vice President of Ambulatory Operations in the Eastern Market, where he oversees regional access initiatives and patient navigation efforts. Prior to this role, he served as Senior Administrative Director for Northwell’s Medicine Service Line, leading Patient Centered Medical Home accreditations, transformations and other quality improvement projects.
